Odontogram by tooth surface, six-site periodontal chart with bleeding and plaque percentages, DMFT from the chart, occlusion, TMJ, anaesthetic LOT.
Cardiology
ECG strip from an attached CSV with calipers, Levine murmur grade, pulses and oedema by side, ejection fraction trend, NYHA.
Gynaecology
Obstetric history, due date and gestational age from LMP, pelvic ultrasound measurements, Bethesda Pap and HPV results, colposcopy cervix map.

Ophthalmology
Visual acuity, refraction, intraocular pressure trend, pupils and motility, slit-lamp and fundus findings per eye, cup-to-disc ratio, spectacle prescription.
Psychology and psychiatry
PHQ-9 and GAD-7 filled item by item and summed, mental state examination, psychoactive substance use, mood and anxiety trend.
Aesthetic medicine
Fitzpatrick phototype, Glogau photoageing, consent, product LOT, injection maps of the face with product, units and LOT per point.
Emergency
Five-level triage priority, wait to assessment, ABCDE primary survey, GCS, interventions, injury and burns maps with summed TBSA, disposition.
Pulmonology
FEV1, FVC and PEF with the FEV1/FVC ratio calculated, breath sounds by zone, lung map, six-minute walk, AHI, pack-years.
Veterinary dentistry
Shared veterinary exam form, with no dental chart yet: mouth and teeth marked normal or abnormal, findings pinned on the body map, treatment given.
